About Forms
You can create all kinds of forms that you can place in any of the pages in your Business Profile.
Here's a few ideas for forms:
- Contact Us - Build this form to allow your visitors to contact you.
- E-mail Newsletter Sign Up - Allow visitors to sign up to receive your e-newsletter.
- Take Survey - Want to know what color of flowers your visitors want to plant in their gardens next spring? Use a form to conduct a survey.
- Sweepstakes & Shopping Sprees - Your visitors can enter to win a sweepstake or shopping spree at your place of business.
- Request A Quote - Allow your visitors to request a price quote for the products or services you provide.

How To Build A Form
Click on the "Forms" tab in your Business Profile Manager.
Then click on the link that says "Click here to add a new form" (shown boxed in red in the image to right).
Setting Up the Form
- Name - Enter a name for your new Form.
- E-mail Distribution List - Enter the e-mail address(es) you want the form submissions to go to. In other words, after a form has been filled out by a visitor to your Business Profile, the information he or she filled on on your form will be sent to the email addresses you provide in this field. TIP: Make sure to seperate multiple e-mail addreses with a comma.
- Confirmation - Type a message in this field to let the person who filled out the Form know that it was submitted. After they have filled out and sent the form, they will see this message pop up on their screen. Example: Thank You for contacting us! A member of our staff will be in touch with you as soon as possible."
- Disclaimer (Optional) - Type any disclaimer in this field.
- Save Changes - Click on the green 'Save Changes' button.
