Add Fields To Your Forms

Label/Name - In this field, type in what will be the first question you will ask in your form. Example: First Name
Required? - If this will be a required field that must be filled out, check the box next to "Required."
Type- Click on the grey drop down menu button and select the type of field you want from the list that appears.

A new form field has been added! The title of this field is Display Priority. This field will appear right below the Required? field as seen in the picture above. The Display Priority Field will insure that the questions in your form will appear in the right order on the live page in your profile. The best way to use this field properly is to design your form on a sheet of paper or in a Word file first, just so you can know how many fields (questions) it will ask. For example, if you are building a Contact form that will ask three questions, such as: Name, Email Address, and Zip Code, you will number these questions starting with the highest number for the first question that will appear on your form. For example, you would enter the number 3 in the Display Priority Field for Name, the number 2 for Email Address, and the number 3 for Zip Code.

Types of Fields
Single Line Input - Single line input fields are good for entries such as First Name, Last Name, E-mail, etc.
Multiple Line Input - Multiple line input fields are good for entries such as Comments, Suggestions, Questions, etc. This allows the person filling out your form to type in multiple lines of text.
Checkbox - A checkbox is good for use when you want to ask a question such as: "Do You Want To Receive Our E-Mail Newsletter?"
NOTE: After you have selected a field type that offers several options, such as a Checkbox List, 'Selectable Options' will appear. Selectable Options will be the answers for a question you are asking on the Form you are creating.
For example: You might want to ask the person who will be filling out your form: "When is the best time to contact you by phone?" The answers to this question might be: Morning, Afternoon, or Evening. To create these answers, type in one of the answers in the "Add" box (shown boxed in red in the picture above) and then click the green Add button. The answer will then show up in the Selectable Options window (shown boxed in purple in the picture above). Now type type the next answer in the Add box, and so on. After having typed in all of the answers, click on the green Add Field button.
Checkbox List -This can be used to ask a question for which there are multiple answers. The user can check all answers that apply.
Radio Button List - This can be used to ask a question that has multiple choices for an answer. Only one answer can be selected.
Drop Down List - This can be used to ask a question where one answer can be selected from a list of answers that appear in a drop down list.
